Quick answer
The instrumental answers кем? чем? (with whom? with what?). Use it after с (together with), for the means of doing something, and after рабо́тать / быть / стать.
Я иду́ с бра́том. Он рабо́тает врачо́м.
I am going with my brother. He works as a doctor.

с or no с?
If you could replace “with” by “together with”, use с. If it is a tool (“with a pen”, “by bus”), Russian uses the bare instrumental — no preposition.

Typical mistakes
Я пишу́ с ру́чкой.
Я пишу́ ру́чкой.
“With a pen” as a tool needs no preposition. С ру́чкой would mean the pen is your companion.
Он рабо́тает врач.
Он рабо́тает врачо́м.
A job after рабо́тать is instrumental.
